I need assistance setting up a MyReid account.
There are multiple ways to sign up for a MyReid account. You can either initiate the process on your own, or you can sign up with the help of staff at your provider's office.
- Patient-led process: If you've received an activation code, you can enter that here to initiate the process. To maintain security of information, activation codes do expire. If your activation code has expired, you can call your provider's office or request a new code from the MyReid Help Line. If your record is on file, a new activation code will be emailed or texted to you depending on your communication preferences.
- If you haven't received an activation code, you can sign up through our secure process through Experian. After you enter the required information on this form and it matches a Reid Health record exactly, you'll receive instant access to a MyReid account. If your process is unsuccessful, please call the MyReid Help Life at (765) 935-8831.
- Reid Health staff-led process: At your next office visit, tell the staff you'd like to sign up for a MyReid account. They'll be able to create an account for you. You'll need to remember the username, password, and verified email associated with the account setup.
I need a new activation code or my activation code doesn't work.
