Your care team includes Certified Diabetes Care and Education Specialists (CDCES) and Registered Dietitian Nutritionists (RDNs) who specialize in diabetes prevention and management. Together, they provide coordinated, patient-centered education grounded in the latest clinical guidelines and evidence-based practices.

Diabetes prevention & education services
We offer a wide range of services designed to support every stage of your diabetes journey.
Services include:
- One-on-one medical nutrition therapy (MNT)
- Comprehensive diabetes self-management education
- Insulin pump training with hands-on instruction and follow-up
- Continuous glucose monitoring (CGM) setup and interpretation
- Free prediabetes and diabetes prevention classes
- Ongoing support to help you make lasting lifestyle changes
Many services require a referral from your primary care provider and are billable to insurance.
Available classes and support
Diabetes prevention class
Our free 1.5-hour Diabetes Prevention Class is designed for individuals with:
- A family history of diabetes
- Prediabetes or insulin resistance
- An interest in improving nutrition and lifestyle habits
This class focuses on practical steps to lower your risk of developing Type 2 diabetes through healthy eating, physical activity, and behavior changes.
Diabetes self-management training program
Our Diabetes Self-Management Training (DSMT) program gives you the tools and confidence to manage diabetes effectively. The program includes:
- A one-on-one assessment
- Four small-group education sessions
Topics covered:
- Blood glucose monitoring and diabetes medications
- Meal planning and carbohydrate counting
- Exercise, stress management, and healthy routines
- Foot, eye, and dental care
- Preventing diabetes-related complications
