Releasing your medical information
Reid Health takes your privacy seriously and protects your health information.
To get copies of your medical records, please fill out the Release of Information Form below. This form must be completed and on file with the Health Information Management (Medical Records) Department before records can be shared.
On the form, check the boxes for the records you need, such as lab results or a discharge summary. If what you need is not listed, write in the details. Be sure to include the dates of service. Requests for future dates cannot be accepted.
After completing the form, submit it to the Health Information Management Department. Please bring a valid photo ID so we can confirm you are allowed to receive the records.
How to request medical records
- Download and complete the Release of Information form.
- Select the records you need and include the date of service.
- Submit the completed form to the Health Information Management (Medical Records) Department.
- Bring a valid ID to confirm your identity.

Who can request medical records?
Medical records may be requested by the following individuals or groups:
- The patient
- A parent or legal guardian, if the patient is under 18
- An authorized representative acting on the patient’s behalf
- A healthcare provider involved in the patient’s care
