Take charge of your family’s healthcare with MyReid
Family life is
often hectic. Trying to keep track of everyone's work, school, sports, and
social schedules can leave your head spinning.
When a family
member needs care -- even if it's just a routine visit -- there's suddenly new
appointments, test results, instructions from your provider, and more to track.
The last thing you need is to have to jump back and forth from one digital
account to another to keep everything straight.
That's where
the MyReid mobile app comes in.
With MyReid,
you can easily get all your family members' care information right within your
own account, thanks to proxy access.
What's proxy access?
The ability to
see another person's medical information within MyReid is called proxy access.
With it, you can view a loved one's upcoming appointments, test results,
immunizations, communications with providers, and more.
How do I get proxy access?
There are a few
different ways to go about getting access to a family member's information.
For
minors ages 13 and younger:
- A parent or legal guardian can request access to their child's account in person at the office of their child's primary care provider.
- The parent/legal guardian must show ID and sign a Consent for Proxy Access form.
For
minors ages 14-17:
- On the child's 14th birthday, the level of access for the parent/legal guardian automatically changes from full to partial to protect the child's privacy because they can now have their own MyReid account.
- Only billing, immunization, and growth chart records are available under partial access. Appointments can be scheduled by a parent/legal guardian, but they won't be able to see their child's upcoming visits.
- A parent/legal guardian can request full access to their child's account in person at the office of their child's primary care provider.
- The parent/legal guardian must show ID and sign a Consent for Proxy Access form.
- For a parent/legal guardian to have full access, the minor must have a private, in-person conversation with their provider and the person wanting full access. Verbal permission must be given by the minor.
Adults
ages 18 and older:
- When a child turns 18, all previously granted access ends as adults can create their own MyReid account and invite others to access their health information.
Diminished
capacity/incapacitated patients
- If a loved one or someone in your care has been determined to be incompetent or incapacitated, you can request access to their account.
- The request must be made in person at the office of the patient's primary care provider. Paperwork showing guardianship, Power of Attorney (POA) for healthcare, or the Appointment of Healthcare Representative must be provided, and the requestor must sign a Consent for Proxy Access form.
How do I give someone else proxy access to my account?
If you'd like
to give someone access to your health information, you can do so in MyReid's
Sharing Hub. Just select the "family member, close friend, or caretaker"
option, then select "Manage ongoing access to your MyReid account," and invite
someone to view your records.
