Deciding Together Advance Directives
There are a million reasons to avoid having the conversation, but it’s critically important. And you can do it.
“Advance Directive” is a general term that describes two types of legal documents: Living Will, and Health Care Power of Attorney. These documents allow you to instruct others about your future health care wishes and appoint a person to make health care decisions if you are not able to speak for yourself.
Reid Hospital’s Deciding Together program allows you to have the conversation now!

Did You Know?
- More than 90% of the people think it’s important to talk about their loved ones’ and their own wishes for end-of-life care
- Less than 30% of people have discussed what they or their family wants when it comes to end-of-life care. (National Survey by The Conversation Project 2013.)
