Reid Health Program of All-Inclusive Care for the Elderly (PACE) supports senior health with comprehensive care to help seniors maintain independence, health, and safety to stay in the comfort of their homes.

Reid Health PACE Center – Care at your PACE

 

The Reid Health Program of All-Inclusive Care of the Elderly (PACE) is designed to keep participants living in the community, preferably in their home, as long as it's safe to do so. A comprehensive team of providers evaluate each participant for medical, functional, social, and cognitive services to help determine what custom care plan might be best to help that participant thrive.

From primary care to rehabilitation services to social activities and more, PACE was designed to help support senior independence. With 24/7 access to a health professional, each day of the year, this is Care at Your Pace.  

 

Who would benefit from PACE?

To be eligible to become a PACE participant, you must:

  • Be 55 years of age or older at the time of enrollment
  • Live in the PACE service area
  • Be eligible for a nursing home-level of care
  • Be able to live safely in the community with assistance from Reid Health PACE

Available PACE services

Through Reid Health PACE Center, each participant has an interdisciplinary team that covers care and services depending on the participant’s individual needs. No two care plans are the same.   

Featured services

Includes physical includes physical examinations, immunizations, skilled RN services, assistance with medication, and more.

Transportation is available to and from the PACE Center and specialty appointments.

Healthy meal options prepared by registered dietitians are delivered to participants’ homes. 

The PACE Day Center offers a TV room, outdoor patio, and a full social calendar for creative activities.
