Reid Health PACE Center provides personalized healthcare services for seniors including medical care, rehabilitation and therapy, transportation, social support and activities, all tailored to each participant’s unique needs.
Senior-focused health services at PACE
Our program offers a range of services, all designed to help seniors maintain their independence and quality of life. From medical and nursing care to home care, transportation, meal delivery, and social activities, every aspect of our PACE services are customized for you. We believe in ensuring each participant – and their support systems – receive the help they need to live comfortably and thrive.
Available health services through PACE
- Primary medical care: Includes physical examinations, immunizations, preventive health, consultations, and women’s health services
- Senior-focused specialty care: Includes audiology, optometry, podiatry, dentistry, and any other care needed in the participant’s care plan
- Nursing care: Includes skilled RN (nurse) services, assistance with medications, and instructions to prevent illness and disability
- Home Health and Home Medical Equipment: Includes light housekeeping to personal care and providing assistance with dressing, bathing, and more. Also, medical equipment can be conveniently drop-shipped to your home, ensuring you receive exactly the items needed for your care.
- Personal care: Includes bathing, hair and nail care, dressing, grooming, and assistance with other hygiene activities

REID HEALTH PACE CENTER
After-hours or emergency care
Care is available to PACE participants 24 hours a day all year long. If a participant or their caregiver needs to speak to a healthcare professional to receive advice or treatment and can’t wait, after-hours support is available.
Urgent care and emergency care is also available for PACE participants if their conditions fall within the definition of an emergency or urgent care situation as identified in the enrollment agreement. Your Reid Health PACE Enrollment Specialist will review this available service in detail.
PACE CENTER
Transportation
PACE offers transportation to and from the Reid Health PACE Center as needed. Transportation to and from specialty appointments and other health appointments is also available through PACE transportation. PACE has seven vans offering transportation to all six counties.

REID HEALTH PACE CENTER
Meal delivery and nutritional counseling
Eligible PACE participants receive a full lunch, along with morning and afternoon snacks. Nutritional counseling is integral to care plans, with meals tailored to specific dietary needs. Reid Health PACE provides fresh, delicious, and culturally familiar meals, ensuring participants can make informed choices about their nutrition.
Choose Well at Home delivers meals to participants’ homes. Prepared by registered dietitians, these calorie-controlled meals are low in saturated fat, sodium, and consistent in carbohydrates.

PACE
Home Health & Home Medical Equipment
At the Reid Health PACE Center, many services are available in the home, tailored to each participant’s needs.
PACE home health services can include:
- Homemaker/chore and personal care services
- Home health aide and physical and occupational therapy
- Home-delivered meals (Choose Well at Home)
- Skilled nursing and medical social services
- Reid Home Medical Equipment
A home visit by the PACE team identifies necessary medical equipment, which is delivered for participants’ safety and independence. This equipment is facilitated through Reid’s Home Medical Equipment (HME) store.
Additional services
PACE offers even more services to support our participants’ overall well-being. Our enrollment specialists can answer questions and connect you with resources that’ll be the best fit for your needs.

Medication management
To make medication management easier, prescriptions are available at Reid Health PACE Center. PACE medication services will provide and coordinate 100% of the needs and management for PACE participants.

Social services
PACE also offers comprehensive social services to help participants achieve optimal mental and emotional health. Services include community resource referrals, behavioral health coordination, care planning, emergency disaster planning and preparedness, and more.
Caregiver support
It’s important for caregivers to know we understand the challenges that come with caring for a loved one. You’re not alone. PACE offers caregiver support and connection with other families in our community with similar experiences.
