Care at Your PACE
The Reid Health Program of All-Inclusive Care of the Elderly (PACE) is designed to help our participants live more independently by offering a wide range of medical, functional, emotional, cognitive, and social services. PACE is designed to keep the participant living in the community, preferably in their home, as long as it's safe to do so. Each PACE participant has a custom care plan designed with services and care staff to cover needed services from primary care to rehabilitation services and more.
Most importantly, Reid Health PACE participants have 24/7 access to a health professional, every day of the year.

Reid Health PACE Eligibility
To meet enrollment requirements for PACE, a participant must:
- Be 55 years of age or older at the time of enrollment.
- Live in the PACE service area.
- Be eligible for nursing home-level of care.
- Be able to live safely in the community with assistance from Reid Health PACE.
